i-card Policies & Procedures
[Printer-Friendly Format]
|
|
|
- Only one ID is issued per person.
- If you leave the university, you must return your i-card or Special User card to the UIC Photo ID Office, not your college or department.
- Do not throw your ID away once your term has ended! If your term is completed or you are no longer employed at UIC you must return the card to the Photo ID Office, not your college or department.
- All IDs issued by the ID Center are property of the University of Illinois.
- Cards that are not returned are subject to a $20 fee.
- You must be an attending student, employee, or an affiliate of UIC verified by i-card database.
i-card Replacement
-
Students, Faculty/Staff, Special Users, or any persons issued an UIC i-card of any kind must present their damaged card to the ID Center. The standard policy for issuing a replacement card is that a $20 replacement fee will be assessed.
Here are the determining factors on whether $20 replacement fee will be assessed:
- If visual inspection shows evidence of the i-card being exposed to any damage that does not involve swiping or normal transference to storage.
- The card must be received without outward mutilation such as holes being punched into it or the magnetic strip cannot be scratched by anything other than vertical swiping.
- If the information on the i-card front is no longer readable, this is not normal wear and tear.
- The card must be issued at least one year ago.
- If the card is replaced, you must take a new picture.
- The replacement fee is waived for status and name changes, provided the previous i-card or Special User card is returned.
- The $20 replacement fee is payable via cash, credit, or debit.
- Please bring valid form of identification with you such as a valid US Driver’s License, US State ID, or current passport with you.
- If the decision made by the ID Center staff requires further review, the departmental coordinator will make a final determination.
