You should report to the Campus ID Center for a new i-card. Once the card is reported lost or stolen, you will be responsible for the replacement fee. The card cannot be reactivated if it is found at a later date. The replacement fee is $20. The UIC Photo ID phone numbers are 312-413-5940 for 1790 SSB and 312-413-5944 for 242 CIU.
